Insurance Information – Please Read

As a reminder, Provo City School District changed provider networks effective September 1, 2018. Not all providers in the previous Med Plus network are contracted under the new Share network.

If you have the insurance through Provo City School District, you MUST check to see that your doctors are participating in the SelectHealth Share Network. If you go to a doctor who is not part of the SHARE network, you will pay for the entire bill.

You can look up doctors and facilities by going to: www.selecthealth.org or by calling customer service for assistance at 1-800-538-5038.
