Wellness Notes
Don’t forget to sign up for your health screening! The health screening is one of the first steps of the Wellness program. Appointments will go fast so go in now and sign up.
If you went to your Primary Care Provider within the last 90 days or plan to please ask your Wellness Manager or Cat Miner for the Health Screening Authorization Form. Return the completed form to receive credit for your health screening.
Also, as a quick reminder, if you want to be eligible for this month’s drawing, send your hiking information to Cat Miner.
