Hi Everyone!

I wanted to update you on our upcoming SightBuddies event scheduled for October 27, 2017. Students will be bused to Utah Valley Eye Center for a comprehensive eye exam. If glasses are prescribed, they will be delivered to the student’s school a couple weeks later (ONE pair of glasses this time). This is all provided free of charge.

Qualified students are defined as children who meet at least one or a combination of the following requirements:

  • Qualify for free or reduced lunch.
  • Have no vision insurance OR have vision insurance but have otherwise been denied access to visual healthcare (parent negligence included).
  • Students with broken or lost glasses may be eligible. Talk to your school nurse for approval.

If you have students who qualify, parents must complete both sides of the attached permission form. There are links to this form on our Health Clerk Resource Page under “Vision”. Please add their names to the “Provo SightBuddies 10/27/17” Google Doc.

All permission forms must be turned in by October 18.
