Outstanding Educator Nominations

The Provo School District Foundation will once again recognize six outstanding individuals in Provo City School District. One administrator, one classified employee and four teachers will be selected and honored at a dinner event on May 10th.

Included is a link to the nomination form.  In the space provided, please answer the four questions in as much detail as possible. Nominations must be received by 5:00 p.m. on Friday, April 5. We realize that this is the week of spring break and hope that you will still use this week to send in your nominations if needed.
