Back to Wellness Health Screening

Wellness screening is a required activity in the PCSD Wellness University. These events will be similar to the health screenings we have had in prior years, but better. We will have stations on physical, mental, financial, social and nutritional wellness.  We are scheduling in hour increments with slots for 40 people per hour. You can come anytime within the hour of your appointment. The dates for the screening events are:

• September 21, 2023 District Office in the PDC 9:00am – 5:00pm
• September 22, 2023 Grandview Training Center on the stage side 9:00am – 5:00pm
• October 5, 2023 Provo High School in the library 1:00pm – 5:00pm
• October 6, 2023 Centennial Middle School in the cafeteria 2:00pm – 5:00pm

If you can also use a PCP (primary care provider) visit as your wellness screening. To receive credit please download the Health Screening Form (PCP) on the intranet under Human Resource, Wellness or you can download it on your WellRight account. Once it is signed, upload the form to your WellRight account. If you do not have a signed form you can use a EOB (explanation of benefits). Your PCP visits can date back to May 1, 2023.
