Employee COVID-19 Leave Procedure

If you have symptoms or confirmed exposure to COVID-19 please do the following:

  • Do not come to school/work
  • Contact testutah.com and follow their directive
  • Notify supervisor
  • Notify benefits office, Erika Halford, ErikaH@provo.edu, or Cat Miner, CatherineM@provo.edu
  • If a test is needed, do not come to school/work until results are received.

For Negative Results

  • Complete Emergency paid sick leave survey
  • Complete self-certification return to work survey

For Positive Results

  • Follow quarantine instructions by agency
  • Complete emergency paid sick leave survey
  • Do not return to school/work until you are symptom free. Complete self certification return to work survey.
